The Bridge Program is designed to help you place dialysis patients effectively and efficiently to the outpatient dialysis clinic or home setting. In addition, we provide the patient and family members’ educational materials for orientation to the lifestyle changes associated with dialysis therapy.

  • Assist with timely completion of documents based on the patients discharge plan, to minimize potential placement delays to the outpatient setting that can include:
    • Lab results, hepatitis antigen B testing, flow sheets, chest x-ray, immunization history, etc.
    • Review the accuracy of documentation for placement
    • Verify receipt of admissions information by the outpatient dialysis provider
    • Provide the patient and family members with educational resources including modality choices (Treatment Options Program™), lifestyle changes, and access options.
