Accounting Manager
The Accounting Manager will manage the monthly accounting close
and the reconciliation of balance sheet accounts, including the timely
resolution of any balance sheet account issues. Supervise the preparation
and timely filing of sales/use and property tax returns for multiple states.
Supervise the preparation of monthly financial statements and reports, including
accounts receivable reports, to ensure that information is accurate, timely and
in compliance with GAAP.

Accounts Receivable Staff
Accountant
Perform general Accounts Receivable accounting duties in compliance with
Corporate Accounting Policies and Procedures and Generally Accepted Accounting
Principals (GAAP).
Participates
in billing group training to maintain concise and coordinated A/R functions
between the billing group and regional accounting office
Respond to inquiries from the
billing groups concerning A/R problems and A/R system procedures
Review A/R transactions
submitted to regional accounting office for processing, including write-offs,

Accounts Receivable Supervisor
The Accounts Receivable Supervisor will be responsible for
ensuring that A/R receipts, write-offs, unreconciled payments and Medicare

with Corporate and Regulatory policy. Supervise preparation and ensure
integrity of monthly A/R entries and reports, including bad debt provision and
reserve, A/R days, revenue analyses, and collection analyses.

Accounts Payable Supervisor
The Accounts Payable Supervisor will supervise Accounts Payable clerks, ensuring
that accounts payable vouchers are properly authorized, adequately documented,
and paid in accordance with corporate policy and procedure. This position
also supervises the check disbursement function, ensuring that checks are
safeguarded and recorded appropriately. Ensure that necessary accounting
entries are prepared, required reports are processed, reconciliation's
performed and records maintained.
EXPERIENCE AND REQUIRED SKILLS
Minimum of 2 years of accounts payable or 2 years business experience with
1-year supervisory experience.
Functional knowledge of accounting, spreadsheet and word processing software a

Acute Coordinator
Provide dialysis services to all hospitalized patients as prescribed by physician(s); ensure the provision of quality patient care in the most efficient manner in
accordance with company policy; provide coordination and supervision of acute care staff.
Ensure all acute treatments are performed in contracted hospitals. Provide patient
education and counseling to new chronic patients and their families during
hospitalization, including options of modality selection. Participate in
meetings/programs pertinent to administrative, medical, technical, and Q.A. review of the
Acute Care Program. Provide hospital specific Q.A. information as appropriate. Provide
on-going in-service education to appropriate hospital staff. Perform written staff
evaluations, based on routine observations of appropriate technique and adherence to

Acute Charge Nurse
Ensure provision of quality patient care on a
daily basis and supervise the direct patient care staff. In this position you will maintain overall shift operation in a safe,
efficient, and effective manner, conduct staff meetings at least monthly or as
needed to keep patient care staff informed of changes in patient needs or
operations to improve delivery of care. Meet
routinely with the Director of Nursing Services to discuss personnel and patient
care status, issues, and information. Assess

Administrative Assistant
Assist in the day-to-day management and operation of designated FMCNA facilities in
order to maintain standards as established by the Company. Assist in the integration of long and short-term business plans for
a designated group of facilities. Assist the Administrator
and Patient Accounts Manager in the accounts receivable function, specifically
concentrating on the area of proper funding information. In conjunction with Chief Technician, insure
maintenance of medical equipment with documentation required by FMCNA
Management and Federal regulations. Participate in personnel activities at the direction of Administrator,

performance evaluation, wage and salary compensation, employee relations, and

This position is responsible for general office and secretarial duties for

Admissions Coordinator
Supports FMCNA's mission, vision, core values and customer
service philosophy. Coordinates incoming referral calls and admissions for
patients in a specific area. Responsible for ensuring required demographic,
financial, referral, clinical, and other admission date is colleted, completed
and communicated per FMCNA policies and procedures. Assists in local marketing
to potential referral sources in accordance with the FMCNA Compliance Program
thus contributing to the growth of the facilities in the area. Adheres to the
FMCNA Compliance Program, and all regulatory and FMS policy requirements.
CUSTOMER SERVICE
Responsible for driving the FMS culture through customer service standards.
Accountable for outstanding customer service to all external and internal
customers, including patients, staff, physicians, Business Unit and Corporate
personnel, and vendors.
Develops and maintains effective relationships through effective and timely
communication with all customers.
Takes initiative and action to respond, resolve and follow up regarding customer

Obtains necessary patient information-clinical, financial and demographic -

Area Manager

running a dialysis business within a defined group of facilities through
effective leadership and management of all aspects of customer service, quality
patient care, marketing of FMS and financial management. Maintains profitability
while adhering to the FMCNA Compliance Program, including following all regulatory
and FMS policy requirements. Responsible for profit and loss management of each
assigned facility/programs including optimal performance of facility operations
to achieve or exceed budgets and key performance indicators.
EXPERIENCE AND REQUIRED SKILLS
At least 2 years experience in managing and leading groups, preferably in
healthcare. Demonstrated leadership competencies and skills for the position, including
excellent communication, customer service, continuous quality improvement, relationship
development, results orientation, team building, motivating employees, performance
management, and decision making. Demonstrated management skills necessary to provide
leadership in the supervision of dialysis personnel and to ensure the delivery of maximum

Assistant Nurse Manager - Renal Research
Institute
Assists the Nurse Manager in the coordination of nursing care on
a shift basis within a specific unit. Assists with the supervision of all
nursing activities related directly or indirectly to patient care. Assists
in coordinating the development, planning, implementation, and evaluation of the
nursing regimen for patients within a given unit. Coordinates staff
activities in the performance of the nursing process. Participates in the
Patient Classification system. Participates in patient care conferences.
Evaluates current and changing needs of the nursing unit and makes
recommendations to the Nurse Manager. Participates in the planning,

Billing Group Supervisor
Responsible for daily operations of the billing group, including staff
supervision and training, office management and other duties under the direction
of the Patient Account Manager. Assign A/R tasks/ functions to Senior
Patient Account Reps. and Patient Account Reps. based upon established job
descriptions. Oversee and supervise responsibilities of Patient Account
Representatives. Ensure that adequate training is provided for new staff
and that on-going training occurs for existing staff so that all A/R functions
are adequately covered. Evaluate the performance of A/R staff and prepare
performance evaluations. Perform all payroll/ personnel record processing
including preparation of weekly payrolls, PTO reconciliations, and payroll
record maintenance forms. Supervise office management, including accounts

Business Unit Compliance Officer
Actively participates in and supports the development, implementation and
evaluation of FMCNAs Compliance Program within the Business Unit or Specialized
Program. Working through business unit management, directs, oversees and monitors the
successful implementation of the Compliance Program of the Business Unit or Specialized
Program, as established by business unit, divisional and/or corporate compliance
departments. Is expected to exercise sufficient independence and autonomy to ensure all
compliance concerns are appropriately assessed and resolved. Oversees compliance with
federal, state and local regulations; payer reimbursement requirements; and Company
policies, procedures and guidelines, supporting the integration of compliance activities

Business Unit Director of Managed Care and Marketing
Supports FMCNA's mission, vision, values and philosophy. Adheres to the
FMC Compliance Program, including following all regulatory and FMS policy
requirements. Directs organization's activities of strategic planning,
evaluation, coordination, and implementation of various managed health care
programs (HMOs, PPOs, POS, etc). Oversees Business Unit's contractual
relationships with managed care organizations and other third party payors
regarding rate structures for services, clinical and financial reporting
obligations, establishment of other business terms per the FMC Guidelines,
contract maintenance such as facility list updates and amendments. Develops and
directs regional marketing efforts to third party payors in order to support

Care Manager-RRI
The care manager serves as the coordinator and facilitator of the
Renal Care Management program. Working under the direction of the Nephrologist,
the Care manager fulfills the program's mission through the coordination of
multiple disciplines on behalf of meeting the needs of the PRE-DIALYSIS and ESRD
patient population. This position, by utilizing the nursing process, assures
that positive patient outcomes are achieved for a respective patient caseload.
Care managers' focus on prevention, health education, coordination of
outpatient services and outpatient encounters that prevent complications and
acute encounters. They track patient demographics, patient satisfaction and
access to the ORC system, utilization of services, quality of care, costs and
clinical outcomes. Care managers are the link between the member, the
nephrologists, the healthcare team, external community providers, the payer and
Claims administration. The care manager is cognizant of the philosophy,
standards, objectives and policies of the facility and Optimal Renal Care.
EXPERIENCE AND REQUIRED SKILLS
Recent one-year experience in nephrology or dialysis nursing.
5 years in dialysis, transplant or nephrology/endocrinology health care
environment.
Care management skills including population oversight, problem solving,
telephone triage, meeting organization and facilitation, referral authorization,
etc. Working knowledge of automated systems. Excellent interpersonal
skills and a patient orientation.
Strong communication and group facilitation skills.
Ability to communicate with all levels of personnel both verbally and in

Case Aide
Provides support for the Social Services Department. Supports the FMCNA
mission, values, and customer service philosophy. Adheres to all
requirements of the FMCNA Compliance Program, and FMS patient care and
administrative policies. Assist social worker (MSW) in obtaining necessary
patient information from referral sources. Assist MSW with patient
admissions process. Maintain copies of all insurance cards in patient
file. Under direction of MSW, assist in obtaining necessary documentation
from patient to complete indigent waiver/payment agreement for patients without
complete coverage and update as necessary. Assist patients with
interpreting bills, notices, forms, etc. Work collaboratively with Billing
and MSW to ensure patient receives accurate information. Assist patients
with travel or transfer to other dialysis facilities. Collect and forward
all necessary paperwork to receiving facilities for transfer or transient
patients. Advise Billing of insurance and/or address changes.
Maintain current file of spend-down information for Medicaid patients.

Case Manager
Assist Home Training Supervisor or DON, in the identification,
evaluation, and training of candidates for the home dialysis program.
Responsible for accurate documentation of
information related to the patients training and treatment. Ensure that the information is documented
appropriately in the individual patient record and entered into the Proton
System. Responsible for ongoing monitoring, assessment and intervention for home patients including: home
visits, clinical appointments, additional training sessions or reviews, lab
studies and annual studies such as x-ray and EKG. Responsible for compliance with patient supply procedures,
appropriate documentation and ensuring maintenance of all durable medical
equipment, i.e., preventative maintenance, water analysis. Perform monthly chart audits and report
results to supervisor. Monitor the Peritonitis Rate and PET Test results. Report the outcomes to the attending physician(s) and the Quality
Assurance Committee. Under the direction of the Home Training Supervisor or DON responsible for making changes
in patient treatment plans in conjunction with the physician. Responsible for scheduling back-up treatment

Centralized Technical Program Manager
Responsible for the overall operation, condition, maintenance, and repair of all
water treatment and medical equipment, as well as mechanical/electrical systems
so as to ensure safety of the patients as well as the staff operating the
equipment throughout the centralized technical program area. Oversees the
inventory control and regulatory compliance functions and ensures suitable
measures are taken to maintain facilities appropriately. Supervises,
schedules, and trains staff in centralized program to ensure effective and
efficient delivery of service to customers/facilities. Supports the FMCNA
mission, vision, values, and customer service philosophy. Adheres to all
the requirements of the FMCNA Compliance program, and FMS patient care and

EXPERIENCE AND REQUIRED SKILLS
Minimum of 3 years of Dialysis Technical/Mechanical supervisory experience.
Successful completion of the FMCNA Training Program for Technical Staff.
Successful completion of Technical Validation Program exam A within 12 months of
new position, completion of entire Technical Validation Program with 24 months
of new position or equivalent.
EDUCATION
Associates degree in electronics, Bio Medical Engineering or equivalent

Charge Nurse
Ensure provision of quality patient care on a daily basis and supervise the

Chief Technician
Responsible for the overall operation, condition, maintenance, and repair of all
water treatment equipment, medical equipment, and mechanical/electrical systems so as to
ensure safety of the patients as well as the staff operating the equipment. Also
responsible for reuse, inventory, and housekeeping. Have knowledge and proficiency in the
initiation and shut down of all equipment in the facility. Have a comprehensive knowledge
of the function and safe operation of back-up water treatment equipment and related
mechanical and electrical systems. Establish and maintain a preventive maintenance program
including support documentation for all water treatment equipment, medical equipment, and
related mechanical and electrical systems.
EXPERIENCE AND REQUIRED SKILLS
Minimum of two years of Technical/Mechanical experience. Successful completion
of the BMA Reuse and Equipment Technician Training Program. Successful completion of
Technical Validation Program exam A within 18 months from date of hire and parts B and C
within 36 months for newly hired employees. For current employees promoted from equipment
technician must have successfully completed the Technical Validation Program. Proven
verbal and written skills. Demonstrated supervisory and leadership skills, including those

Clinical Manager
Support FMCNAs mission, vision, core values, and customer service

Dialyzer R&D Product Development Manager
Under general supervision of Dialyzer R&D Manager engineering group manager

(ESRD). Act as primary point of contact and leader for project teams in
relation to managed products. Develop detailed engineering specifications for

Home Care Nurse
Provide education and follow-up to patients selecting the option of
home dialysis. Provide pre-ESRD education to all patients requiring renal

nursing care and education of patients with ESRD who have elected Home Dialysis
Services. Responsible for educating the patient on all aspects of home
dialysis, including end stage renal disease, diet, medications, dialysis
concepts, how to perform procedures, expected results, and any complications
which may occur. Assess that patient has learned the above by proper
demonstration and/or verbal response before release from formal training.
Function as a Modality Selection Consultant. Responsible for making
adjustments and modifications to the dialysis treatment as indicated or

information related to patient treatment. The information is documented
appropriately in the individual patient record. Responsible for reporting
any significant information or change in patient condition to the Home Dialysis
Services Supervisor (if applicable), physician, social worker, and/or dietitian,

patient via telephone communication.

Home Training RN
Assist Home Training Supervisor or DON, in the identification, evaluation, and
training of candidates for the home dialysis program. Responsible for
accurate documentation of information related to the patients training and
treatment. Ensure that the information is documented appropriately in the
individual patient record and entered into the Proton System. Responsible
for ongoing monitoring, assessment and intervention for home patients including:
home visits, clinical appointments, additional training sessions or reviews, lab
studies and annual studies such as x-ray and EKG. Responsible for
compliance with patient supply procedures, appropriate documentation and
ensuring maintenance of all durable medical equipment, i.e., preventative
maintenance, water analysis. Perform monthly chart audits and report
results to supervisor. Monitor the Peritonitis Rate and PET Test results.
Report the outcomes to the attending physician(s) and the Quality Assurance
Committee. Under the direction of the Home Training Supervisor or DON
responsible for making changes in patient treatment plans in conjunction with
the physician. Responsible for scheduling back-up treatment when necessary

Position reports to the Director of Home Therapies and is responsible for

Regional Secretary
This position is responsible for providing administrative support of the
Regional Vice President. Duties include general clerical/secretarial and
project based work: Prepares correspondence, reports and other materials
such as presentations, as required. Complete forms and reports as required by
the Regional Area and Corporate offices as well as State and Federal agencies.
Performs administrative support tasks that include, but are not limited to,
typing, word processing, using computer applications, mailing, faxing, copying
and filing. Answers and responds to telephone calls in an appropriate
manner. Creates, transcribes and distributes meeting agendas and minutes.
Sets up travel and meeting arrangements for the Regional Vice President and
other management personnel. Maintains the Regional Vice President's
calendar - scheduling appointments and maintaining itineraries. May
coordinate placement of advertisements for vacant facility positions.
Maintains and updates attendance records. Creates and maintains Excel and
other databases as well as organizational charts. Participates in
project-based work. Maintains inventory of office forms and supplies.
Reconciles monthly statements/invoices before forwarding to Regional A/P
department for payment. Maintains other expense records. Assists new
facilities during their initial set up and transition process - forwards
necessary supplies and forms. Reviews and ensures compliance with the Code
of Ethics and Business Conduct and all relevant Company and Business Unit
policies and procedures, and local, state, and Federal laws and
regulations.
EXPERIENCE AND REQUIRED SKILLS
Minimum two years of
secretarial or business experience with good typing, word processor and computer
skills.
Knowledge of Microsoft Word, Excel (or other spreadsheet software), and
PowerPoint preferred.
EDUCATION
High School diploma or equivalent.
Secretarial school preferred.
Regional Vice President
Supports FMCNAs mission, vision, core values and customer service
philosophy. Responsible for running and growing a dialysis business within a defined
region, through effective leadership and management of all aspects of customer service,
quality patient care, marketing of FMS, and financial management. Maintains profitability
and growth, while adhering to the FMCNA Compliance Program, including following all
regulatory and FMS policy requirements. Responsible for profit and loss management of
facilities/programs within the assigned region, including optimal performance of facility
operations to achieve or exceed budgets and key performance indicators. Trains and
supports Area Managers regarding business goals and practices. Responsible for achievement
of business plan, budget and key performance indicators and communication of same, to Area
Manager, Clinical Manager, facility personnel and Medical Director as appropriate.
Responsible for the development and implementation of a regional marketing plan to support
regional growth and profitability, including facility renovation and expansion, new
facility start-ups, identification and integration of acquisitions, and marketing of FMS
services to potential referral services, in conjunction with the Business Development and
Marketing Departments.
EXPERIENCE AND REQUIRED SKILLS
At least 5 to 8 years experience in managing a business and leading large
groups, preferably within a multi-facility healthcare network. Demonstrated leadership
competencies and skills for the position, including excellent communication, customer
service, continuous quality improvement, relationship development, results orientation,
team building, motivating employees, performance management, and decision making.
Demonstrated management skills necessary to provide leadership in the supervision of
dialysis managers and personnel and to ensure the delivery of maximum quality care to all
patients. Must complete FMS Management and Leadership training modules and ongoing
training and development programs within the specified time line.
EDUCATION
Bachelor Degree in healthcare or business related field.
Advanced business education preferred.
Regulatory Compliance & Inventory
Technician
Under supervision responsible for monitoring and maintaining inventory to ensure
appropriate and adequate supplies for assigned facilities within the centralized
program. Assists in ensuring that OSHA and other regulatory requirements
are met. Responsible for cost containment with regard to inventory control
and maintenance of supplies. Supports the FMCNA mission, vision, values,
and customer service policy.
EXPERIENCE AND REQUIRED SKILLS
At least six months experience working in an
accounting/inventory setting.
Must demonstrate organizational and leadership skills, including those required
to problem-solve in a medical, health care environment.
EDUCATION
High school/vocational diploma or G.E.D., Technician training or
AA Degree preferred.
Computer skills and knowledge of accounting principles preferred.
Renal Dietitian
Responsible for providing ongoing care for all nutrition aspects of pre ESRD,
ESRD, PD and transplant patients. Responsible for comprehensive nutrition
assessment including short term care plan and long-term program within one month
of admission. Recommend appropriate feeding methods/formula or meal patterns.
(i.e. IDPN, supplements). Responsible for developing or obtaining patient
nutrition educations materials. Counsel patient, family, significant other or
caregiver on prescribed diet and monitor adherence and response to therapy.
Review monthly lab work on an individual basis and intervene as appropriate in
collaboration with patient's physician.
EXPERIENCE AND REQUIRED SKILLS
3 years experience in either renal nutrition or diabetes education.
Effective oral communication skills are essential.
Ability to travel with valid drivers license.
EDUCATION
Registered Dietitian.
Current state licensure if applicable.
Prefer membership to the American Dietetic Association and/or professional membership to a
renal association i.e. CRN.
RN Educator
Assist in the support and delivery of all company policies and procedures.
Promote and assist with compliance to OSHA programs in order to maintain a safe
and clean working environment.
Provide training in safe, effective delivery of patient care that assures
compliance with standards outlined in the facility procedure manual as well
as regulations set forth by state and federal agencies. Participate in a program
of education for all facility personnel. Participate in direct patient care as
necessary.

Related
Schedule orientation for all direct patient care staff, reuse technical staff,
and other facility personnel as required.
Assist in the development of instructional materials to assist new personnel in
becoming oriented to facility and operational techniques. Determine the
effectiveness of orientation materials and procedures through direct
observation, and in collaboration with the Inservice Coordinator or Director.
Assist in the annual evaluation and revision of the facility procedure manual as
necessary.
Conduct special inservice classes for facility personnel to implement new
procedures, policies, equipment, and patient care techniques.
Conduct enrichment classes for members of the facility patient care staff.
Plan, implement, and evaluate patient education programs in the facility.
Maintain an annual program for Employee CPR Certification.
Utilize cost-efficient measures with all training supplies.
Maintain library of books and educational materials for facility personnel.
Assist in the evaluation of inservice needs for the clinical staff.
EXPERIENCE AND REQUIRED SKILLS
Minimum of one year hemodialysis experience.
Acute care nursing (ICU-CCU-ACUTE dialysis) experience preferred but not
required.
